The fighting, which broke out suddenly on April 15, 2023, between the paramilitary Rapid Support Forces and Sundanese Armed Forces, is Sudan’s third internal war – and has exacerbated the humanitarian crisis the region was already facing.

More than 220,000 people have crossed the borders. Without a ceasefire, it will get even worse as a protracted crisis is in the making. UNHCR projects that this number could reach 860,000 as conflict escalates.

Education Cannot Wait’s Executive Director Yasmine Sherif came face-to-face with the effects of the brutal conflict during a recent high-level field mission with UNHCR, UNICEF, the Jesuit Refugee Service, and local partners to the border regions of Chad and Sudan, where they witnessed the impacts of the war. In these remote places, large numbers of incoming refugees – a majority of women and children – have settled in flimsy temporary homemade tents. Children are particularly vulnerable and urgently need the protection and support that emergency education interventions provide.

“What we saw is appalling, a heartbreaking dire situation growing very fast. In just two days, the number of refugees grew from 30,000 to 60,000, and 70 percent of them were school-age children. But I am encouraged by the commendable work that UNHCR is doing on the ground.”

The UN’s global fund for education responded with speed to the escalating Sudan refugee regional crisis by announcing a new 12-month USD 3 million First Emergency Response grant. Sherif says this is a catalytic fund to help UNHCR and its partners, in close coordination with Chad’s government, kickstart a holistic education program.

Before the new crisis erupted in Sudan and despite Chad being one of the poorest countries in the world, Chad was already hosting Africa’s fourth largest refugee population.

“Chad is second to last on the Human Development Index, only before South Sudan. The government of Chad is showing very progressive policies and generosity. They have very little resources, and yet they still receive refugees and provide them with much-needed security,” she observes.

Sherif lauded the government’s progressive policy on refugee inclusion within its national education system, stressing that it serves as a model example for the whole region. The new grant brings ECW’s total investments to support vulnerable children’s education in Chad to over USD 41 million. ECW and its partners have reached over 830,000 children in the country since 2017, focusing on refugee and internally displaced children, host communities, girls, children with disabilities, and other vulnerable children.

Funding is urgently needed and critical to implement the regional refugee response plan, which includes an estimated cost of USD 26.5 million for education. While Sudan shares borders with seven countries, including the Central African Republic, Chad, Egypt, Ethiopia, Eritrea, Libya, and South Sudan, nearly all of them are dealing with protracted crises or effects of years of a protracted crisis and require urgent funding to meet the needs of refugees.

“The refugees we met in eastern Chad are in a dire situation. They fled their homes with barely anything and are in very remote and hard-to-reach areas where infrastructures are scarce, and temperatures rise above 40 Celsius. Without emergency relief from international organizations such as UNHCR and UNICEF, it would be difficult for them to survive for long,” she explains.

Despite the government’s best efforts, Chad is dealing with multiple successive shocks, such as climate-induced disasters, large-scale internal displacement, and the Lake Chad and Central African refugee crises, which have eroded the delivery of basic services.

“ECW has made various investments in Chad, including a multiyear resilient program for vulnerable refugee and internally displaced children and their host communities, and other marginalized children in Chad, that has been going on for three years and will be renewed next year. We have also provided USD 2 million in response to the floods or climate-induced disasters affecting Chad,” Sherif says.

“We are now providing this catalytic USD 3 million funding to help UNCHR to provide immediate access to holistic education to the new cohort of refugees arriving from Sudan. ECW’s holistic support enhances school infrastructure and provides school feeding, quality learning materials, mental health, psycho-social services, teachers’ training, and inclusive education approaches. We hope this will inspire other donors and contributors to meet the remaining financing gap.”

Chad’s education performance indicators are among the lowest in sub-Saharan Africa, with 56 percent of primary school-aged children out of school.

UNHCR and its partners in Chad require USD 8 million to implement the education component of the regional refugee response plan. EWC has provided about 40 percent of the budget; the international community should assist with the remaining 60 percent. Sherif hopes that additional support will also be forthcoming for UNICEF and partners to cater to the host communities, who also need support to access quality education.

Incoming refugees live in precarious conditions, lacking the most basic facilities, and need urgent assistance and empowerment. As conditions become increasingly dire, ECW funding will provide access to safe and protective learning environments for incoming refugee girls and boys and support the host communities.

The depth and magnitude of this conflict on children and adolescents are such that their learning and development will most certainly be impaired if immediate access to education is not provided. ECW support offers an opportunity for holistic education to mitigate the debilitating long-term effects of war on young minds.

Fleeing children and adolescents will need immediate psycho-social support and mental health care to cope with the stress, adversity, and trauma of the outbreak of violence and their perilous escape. They will need school meals, water, and sanitation.

“To the international community, we must act now. This is a moral issue; we must prioritize and show solidarity. Our support must be generous. The world cannot afford to lose an entire generation due to this senseless conflict,” Sherif stresses.

ECW and its strategic partners are committed to reaching 20 million crisis-impacted children and adolescents over the next four years. To this end, ECW seeks to mobilize a minimum of USD 1.5 billion from government donors, the private sector, and philanthropic foundations.

Better Mapping of Erosion Risk Areas Needed More Than Ever — Global Issues

Many villagers living along major rivers in Western Rwanda have been among the victims of river erosion and flooding every year.

Felicita Mukamusoni, a river erosion survivor in Nyundo, a mountainous village from Western Rwanda, told IPS that “parts of this village have been eroded to such an extent that we cannot even imagine.”

“I reared cows and goats. My beautiful house was destroyed. The river has taken everything,” she said.

Latest Government estimates indicate that at least 135 people died, and one is still missing following recent flooding and landslides triggered by heavy rains that hit western, northern and southern provinces earlier this month.

In a recent assessment, experts found that land in high-risk areas is mainly used for agriculture, and 61 percent was for seasonal crops. It said that seasonal agriculture exposes soil to splash erosion and further detachment as land is not permanently covered.

The 2022 report on the State of Soil Erosion Control in Rwanda indicates that the erosion control techniques across high-risk areas in Rwanda are still very low.

Erosion control mapping shows that of the 30 districts of Rwanda, land under high erosion risk is about 1,080,168 hectares (45 percent of the total provinces land, which is estimated to be 2,385,830 hectares) of which 71,941 hectares (7 percent of the total risk areas) are at extremely high risk.

According to the same report, at least 190,433 hectares of land are considered very high risk (18 percent), 300,805 hectares are at high risk (28 percent), and 516,999 hectares (48 percent) are at moderate risk.

Dr Charles Karangwa, a climate expert based in Kigali, told IPS that It is unfortunate that fresh disasters happened again despite a lot of investment in the past.

“Rwanda needs to explore other complementary solutions such as water management infrastructure, water harvesting, and where possible, relocate those living in highly risky areas to allow nature to regenerate will help to stabilise the situation both in the long term and medium term,” he said.

Apart from being highly populated, Karangwa pointed out that there is quite a link with geographical vulnerability because of soil erosion risk, which is worsened by high population, and this increased pressure on land.

Flood Management and Water Storage Development Division Manager at Rwanda’s Water Resources Board (RWB), Davis Bugingo, told IPS that among solutions to cope with recurrent disasters in Western Rwanda is the establishment of flood control infrastructures to regulate water flow and reduce flooding risks.

These include the construction of the neighbouring Sebeya retention dam, and Gisunyu gully rehabilitation works expected to significantly contribute to reducing flood impacts in the region.

While accurate and up-to-date data on river flow, topography, and flood vulnerability remains crucial for effective flood management, Bugingo observed that limited data availability and quality could pose challenges in accurate flood forecasting, risk assessment, and planning.

Apart from land use, which contributed to increased flood risks, experts observed that constructions in flood-prone areas, encroachments on riverbanks, and inadequate zoning regulations had exacerbated the impact of floods and hindered effective flood management efforts in western Rwanda.

Most recently, RWB has developed a dedicated application to collect more information to inform future analysis, relocation of people living in risky areas, and adjusting tools used to design flood control infrastructure.

The above tool provides information on flood exposure and areas at risk that can be visualised in 3D and shared the information with the public or other organisations. However, experts are convinced that despite these innovative solutions, limited financial resources may hinder the implementation of these large-scale infrastructure projects, such as dams, flood control structures, gully reclamation and drainage systems.

Rwanda is one of Africa’s most densely populated countries, with large concentrations in the central regions and along the shore of Lake Kivu in the west. This East African country’s total area is 26,338 km2, with a population of 13,246,394.

Bugingo points out that inadequate land use still contributes to increased flood risks.

“Constructions in flood-prone areas, encroachments on riverbanks, and inadequate zoning regulations continue to exacerbate the impact of floods and hinder effective flood management efforts,” he said.

Indian Christians Seek Equal Rights for Dalit Converts — Global Issues

Her husband, Subhash Kumar, sells the handmade brooms she makes from trees in the open market to earn a living. Living in makeshift hutments, Kumari’s family’s meagre income makes it difficult to make ends meet.

In the original Hindu social structure, the Dalits had the lowest social standing, and they continue to be regarded as being so impure in the majority of the states that caste Hindus view their presence as contaminating. Many Hindus consider their vocations debasing, such as dealing with leather, night soil, and other filthy work, which accounts for their unclean status in society.

Kumari has two children who study in a nearby government school, and she wants them to receive an education and eventually earn a good living. However, Kumari says that society and the government leave her family in dire straits because of their Christian faith. She believes that Dalits who practice other religions receive government grants, health and education benefits, and reservations in government jobs, but as Christians, they are overlooked.

Despite being economically disadvantaged, Kumari’s family does not qualify for government schemes. Her husband, Subhash Kumar, says that they earn no more than 5000 rupees (USD 80) a month and providing their children with a good education is challenging without government support. Dalit Christians are discriminated against and denied benefits solely because of their faith, adding to their struggles.

Background of Discrimination

After India gained independence from British rule in 1947, the government introduced significant initiatives to uplift the lower castes. These initiatives included reserving seats in various legislatures, government jobs, and enrolment in higher education institutions. The reservation system was implemented to address the historic oppression, inequality, and discrimination experienced by these communities and to provide them with representation. The aim was to fulfil the promise of equality enshrined in the country’s constitution.

On August 11, 1950, the President of India issued the Constitution (Scheduled Castes Order, which provided members of Scheduled Castes with various rights as outlined in Article 341(1) of the Indian Constitution. However, the third paragraph of the order stated that “no person who professes a religion different from Hinduism shall be deemed to be a member of a Scheduled Caste”.

In 1956, Dalit Sikhs demanded inclusion in the Constitution (Scheduled Castes) Order, 1950 and were successful in getting listed in the Presidential SC/ST Order, 1950, through an amendment to Para 3 of Article 341. Dalit Buddhists were also included through an amendment to Para 3 of Article 341 in 1990.

Christians and Muslims of Dalit origin now demand that they get social welfare benefits meant to uplift Dalit people. Both communities have been denied these benefits since 1950 because the government says their religions do not follow the ancient Hindu-caste system.

Legal angles

Nearly 14 Christian organisations in India have filed petitions in the country’s Supreme Court requesting reservations in education and employment for the 20 million Dalit Christians, who account for 75 percent of the total Christian population in India. In India, people are segregated into various castes based on birth, and 80% of the population is Hindu. Although parliament outlawed the practice of untouchability in 1955, India’s lower castes, particularly Dalits, continue to face social discrimination and exclusion.

In April this year, the Supreme Court of India requested that the federal government take a stance on granting reservation benefits in government jobs and educational institutions to Christian converts among the Dalits. The court is scheduled to hear the petition and decide on the status of Dalit Christians.

The Indian government had formed a committee to investigate the possibility of granting Scheduled Caste status to those who had converted to other religions but claimed to have belonged to the community historically. This was the second panel set up by the government after it rejected the recommendations of the first commission, which had recommended including them.

According to Tehmina Arora, a prominent Christian activist and advocate in India, it goes against the core secular values of the country to deny rights to individuals solely based on their religious beliefs. Arora emphasised that even if individuals convert to Christianity or Islam, they continue to live in the same communities that treat them as untouchables, and their circumstances do not change. Therefore, she believes people should not be denied the benefits they previously had due to their faith.

God is Our Hope

Renuka Kumari shares that she prays for her children’s success every day, hoping that God will help them excel in life. She laments that their entitlements are denied solely because they chose Christianity as their faith. She finds it ironic that they are denied government grants for this reason, causing them to live miserable lives and struggle every day to provide their children with education and a better future. Kumari’s two children, Virander and Prerna, are currently in the second and seventh grades. Sujata aspires to become a teacher one day and is passionate about mathematics. She dreams of teaching at her school, just like her favourite teacher, and is particularly fond of algebra.

Acute Hunger an Immediate Threat To Over a Quarter of a Billion People — Global Issues

According to the latest Global Report on Food Crises (GRFC), the number of people experiencing acute hunger, meaning their food insecurity is so bad it is an immediate threat to their lives or livelihoods, rose to around 258 million people in 58 countries and territories in 2022.

That was an increase from 193 million people in 53 countries and territories in 2021 and it means that the number of people requiring urgent food, nutrition and livelihood assistance has increased for the fourth consecutive year.

It is important to stress here that we are not talking about the number of people around the world who are hungry – a figure that is far higher. Every July the United Nations gives an estimate of the number of people experiencing chronic hunger, meaning they do not have access to sufficient food to meet their energy needs for a normal, active lifestyle, in The State of Food Security and Nutrition in the World (SOFI) report and last year’s, referring to 2021, put the figure at 821 million.

The GRFC report, on the other hand, regards only the most serious forms of hunger.

It said that people in seven countries experienced the worst level of acute hunger, Phase 5, at some point during 2022, meaning they faced starvation or destitution. More than half of those people were in Somalia (57%), while such extreme circumstances also occurred in Afghanistan, Burkina Faso, Haiti, Nigeria, South Sudan and Yemen.

The report said that around 35 million people experienced the next-most-severe level of acute hunger (emergency level, Phase 4) in 39 countries, with more than half of those located in just four – Afghanistan, the Democratic Republic of the Congo, Sudan and Yemen.

The rest of the acute-hunger sufferers were Phase 3, crisis level.

The 258 million figure is the highest in the history of the report and the situation is getting even worse this year

“More than a quarter of a billion people faced acute food insecurity in 2022 – a year that saw the number of people facing food crises rise by a third in just 12 months,” James Belgrave, a spokesperson for the United Nations World Food Programme (WFP), which is part of the Global Network Against Food Crises (GNAFC) that publishes the GRFC report, told IPS.

“And if we look at how 2023 has gone so far, we see that a staggering 345 million people are facing high levels of food insecurity in 79 of the countries where WFP works.

“This represents an increase of almost 200 million since pre-pandemic levels of early 2020, highlighting just how rapidly the situation has worsened.

“As the World Food Programme marks its 60th anniversary in 2023, we find ourselves in the midst of the greatest and most complex food security crisis in modern times”.

Indeed, the GRFC report has only been published for seven years but it has already documented a big increase in the number of people suffering the worst forms of hunger in that time. The number of people experiencing Phase 3 hunger or above was less than half its current level, at 105 million, in 2016.

In 30 of the 42 main food-crisis situations analysed in the report, over 35 million children under five years of age were suffering from wasting or acute malnutrition, with 9.2 million of them had severe wasting, the most life-threatening form of undernutrition and a major contributor to increased child mortality

Although some of the growth in the severe-hunger figure in the latest GRFC report reflects an increase in the populations of the countries analysed, the fact that the proportion of people in those countries experiencing acute food insecurity increased to 22.7% in 2022, from 21.3% in 2021, demonstrates that the situation is getting significantly worse regardless of demographic factors.

The report said that the main drivers of acute food insecurity and malnutrition were economic shocks, conflict and extreme weather events, which are increasing because of the climate crisis.

It said economic shocks were the biggest drivers last year, although the lines between these factors are blurred as all three affect each other, with climate change feeding conflict, for example, and conflict leading to economic shocks.

In 2022, the economic fallout of the ?OVID-19 pandemic and the ripple effects of the war in Ukraine were major drivers of hunger, particularly in the world’s poorest countries, mainly due to their high dependency on imports of food and agricultural inputs.

The central problem is that much of the world’s population is vulnerable to such extremal shocks, in part because efforts to bolster the resilience of poor small-holder farmers in rural areas and fight food insecurity have proven insufficient.

The report says nations and the international community should focus on more effective humanitarian assistance, including anticipatory actions and shock-responsive safety nets, and scale up investments to tackle the root causes of food crises and child malnutrition, making agrifood systems more sustainable, resilient and inclusive.

“The global fight against hunger is going backwards, and today the world is facing a food crisis of unprecedented proportions, the largest in modern history,” Belgrave said.

“Millions of people are at risk of worsening hunger unless action is taken now to respond together – and at scale – to the drivers of this crisis.

“Life is getting harder each day for the world’s most vulnerable and hard-won development gains are being eroded.

“WFP is facing a triple challenge – the number of acutely hungry people continues to increase at a pace that funding is unlikely to match and the cost of delivering food assistance is at an unprecedented high because food and fuel prices have increased.

“In countries like Somalia, which have been on the brink of famine, the international community, working with government and partners, has shown what it takes to pull people back.

“But it is not sufficient to just keep people alive, we need to go further, and this can only be achieved by addressing the underlying causes of hunger and focusing on banishing famine forever.

“We must work on two fronts: saving those whose lives are at risk while providing a foundation for communities to grow their resilience and meet their own food needs”.

Social Media Mobile Phone Data in Disaster Management and the Implications That It Has on Vulnerable Populations — Global Issues

Since its conception in 2015 there have been advancements in the availability of tools such as the use of social media and mobile data which will allow citizens to be at the forefront of disaster management decision-making.

As social media has cemented its permanent spot in society, it is integral that emergency management sees social media and mobile phone data as an asset that can aid in all phases of the disaster cycle.

The widespread adoption of mobile phones and social media platforms has made it possible for people to share information about disasters in real-time, which can help emergency responders to better understand the situation on the ground and respond more effectively. There is a tendency for the public to turn to social media to share information or seek information during a disaster, including sharing posts, requesting help, and sharing the status on critical infrastructure.

Social media can also be used to push out messages from emergency officials to quickly communicate with a large audience and coordinate relief efforts.

There are some mobile applications that are used to identify areas of need and direct resources. With the increasing use of social media, it is important to consider the ethical and practical considerations on using these tools, particularly for vulnerable populations. Access to social media and mobile data is not universal, leaving out some of the most vulnerable communities. There are also concerns about privacy and misinformation in a time where communication channels are already strained.

Hurricane Dorian, South Carolina

First, we will look at an example of Hurricane Dorian and how it hit South Carolina. Hurricane Dorrian was a very powerful category 5 hurricane which had hit the Bahamas and was for them the most intense one on record. It also went on to be the most powerful Atlantic hurricane on record with winds as high as 185 mph. It impacted also on the Virgin Islands and Puerto Rico before landing in the United States.

A number of US states Florida, Georgia, North and South Carolina, and Virginia prepared for its arrival by declaring a state of emergency.

The South Carolina Emergency Management utilized their Twitter and Facebook to spread messaging both before the hurricane and during it about weather and related updates. This worked well because news outlets knew which social media messaging to follow, and they knew the credibility of that information.

Where South Carolina Emergency Management ran into some issues was when it came to private citizens also utilizing social media tools such as twitter who would then tweet at the emergency managers calling for help.

The local first responders didn’t know who had received that information and if telecommunications had already received a call, creating a sense of confusion. Additionally, South Carolina Emergency Management didn’t have the capacity to help with the influx of volume that they had through this new social media messaging capabilities.

There is an interesting opportunity here for improved Disaster Response because of three factors.

Firstly, there are new ways of collecting data. Data mining techniques have been revolutionizing every sector of society, and Emergency Management is not an exception to this. We live in an age of big data and there’s an opportunity for transformative change in disaster management because better decisions can be made due to this influx of data.

As a society, we are transitioning from an era of data scarcity to an era of data abundance, and you can even see this in lower- and middle-income countries where we’re now able to gather data in areas that were otherwise relatively data scarce. This is coupled with climate change which is increasing both the frequency and intensity of natural disasters.

Currently, the annual number of people killed from natural disasters is around 60,000 and that is expected to increasingly rise due to climate change – in many cases these are the most vulnerable in society. To help address this, there is a growing focus on a shift to a social perspective to disaster management. This can be best expressed as how and with which tools do we support the most vulnerable when a disaster occurs.

There are three types of data relevant to the discussion.

The first type of data is directed data and that’s operator focused data capturing technology on a person or place. When you think of directed data, you are likely to think of traditional surveillance data cameras and remote sensing.

Automated data is collectively or possibly collected through the normal operations of a system. You can think of mobile phone use like call records, web searches and credit card use.

Lastly you have volunteered data, and that’s data that’s actively or passively produced by citizens. That is looking at crowdsourcing data and social media data which are very rich because it can tell you a lot of information beyond just an individual level.

Looking at a couple different uses of social media and mobile phone data in disasters.

During Hurricane Harvey (2017), a picture of many elderly individuals in a flooded nursing home was tweeted by a man named Timothy McIntosh that lived in Florida. This is the first time that we’re able to see social media being used due to an overrun 9-1-1 system. Citizens turned to Twitter to reach out for help because they couldn’t get in contact with traditional telecommunications.

This picture was tweeted and then after about 2,000 likes and many retweets, Emergency Management officials began evacuating these 18 people in this nursing home, and after every 30 minutes the emergency officials were tweeting at Timothy McIntosh or privately messaging him letting him know about the status updates with this nursing home. However, there is concern of who’s using Twitter to reach out in emergencies. In some studies, there is concern that the Twitter users are typically white male, more educated and living in urban areas.

This began a broader conversation of who’s getting left out through using this means of emergency response messaging.

A different approach – SMS data

Looking at open-source two-way SMS data and there’s two different platforms that will be discussed.

Frontline SMS is interesting because you don’t need a lot to get started. All one needs is power, the internet, a computer that can be used for a hub, a SIM card and then free software and it’s able to turn a laptop into a central communication hub to facilitate messaging. This relies on a text messaging service which is useful because it is easier and more accessible.

In a pilot, Frontline SMS partnered with Strengthening Participatory Organizations in Pakistan following monsoon flooding. They use Frontline to both receive and send messages about complaints or requests for help.

They were also able to receive responses and requests for help. To enable this to be a proactive effort, volunteers had to go out before the disaster to the communities and explain how they wanted them to use this number to text.

What they would receive was information from the individuals including their names, contact information and their addresses so when these individuals message this number it would pop up information about them and better help the response when they would send responders out to those areas. The messages were converted from Arabic into a numbering system, so it was easier to categorize.

RapidPro SMS is another program that was developed by the UN Childrens Fund (UNICEF). It was originally used for faster delivery of blood sample testing, but it’s turned into flexible and customizable software that can be used, with the most common application being in education systems.

However, there is broad applicability for disaster response. Rapid Pro SMS was used for early flood warning systems to send audio messages in Cambodia. They decided to use audio messages because of literacy challenges in the area. The program currently covers over 200,000 households.

Crowdsourcing Data

These are two different projects that are interrelated. The first one is Mission 4636 which is a number that people could use where they would report something that they saw requiring urgent attention. It was used during a 2010 earthquake in Haiti. People would text this number, and that information would then be translated, categorized, and geo-located. Then you could extract this missing person information, so responders knew where to respond.

However, an issue that they had with it was that it was a one-way system. People would say that they needed help, but there was no way of knowing when this aid would actually come or how the message was being received. If the responders needed more information, they couldn’t text back that number and get that extra information they needed.

The Ushahidi project originally began because of violent incidents following a Kenyan presidential election, but now it’s been applied to natural disaster responses. Volunteers will put SMS data, emails and web-based submissions onto a map for the general public to actually see what incidents are happening in their area and they can click there for more information. It would be used to coordinate with responders to go to those specific areas.

Which subsequently ran into a problem with citizens sending the information, it would be translated and posted in English. The populations that they were trying to serve didn’t speak English, so there was a big gap in who could actually use it, and the people that were sending out the messages couldn’t even understand their own messages they put onto this platform.

The Ushahidi map can also scrub Facebook and Twitter, so they could automatically put tweets and Facebook posts onto the map to see those, as well. They realized that there were five key traits that made this platform possible.

  1. The technology was simple.
  2. It was accessible in areas that had low connectivity.
  3. It was accessible by many different platforms so that you could use your phone or your laptop.
  4. There’s an emphasis on the verification of information.
  5. The mixed funding sources also helped it be successful.

There are many implications that these different platforms have for vulnerable populations. First, it is foundational to understand the US Federal Emergency Management Agency – FEMA’s definition of vulnerable populations because there’s so many different definitions. FEMA defines vulnerable populations as:

“a population whose members may have additional needs before, during, and after an incident in functional areas including, but not limited to, maintaining independence, communication, transportation, supervision and medical care. Individuals in need of additional response assistance include those who have disabilities who are from diverse cultures who had limited English proficiency, who are non-English speaking and who are transportation disadvantaged.”

This is a very broad definition, but it is a useful one to use here because all of these populations that are listed would be affected by the use of the different platforms noted here.

Also, in the USA there’s also the Americans with Disabilities (ADA) toolkit which can be helpful in accessibility during disaster management.

Chapter seven of their toolkit is about emergencies and disasters, and under that there’s a requirement that officials make notification systems accessible to people with disabilities. There is an opportunity to incorporate these platforms of open communication, not just notification systems to be under that guidance.

Problems with Social Media and Disaster Management for Vulnerable Communities

The lack of trust that exists in some of these populations that are considered vulnerable is very important to understand. There are historical incidents where their trust has been violated. A lot of vulnerable populations do have a lack of trust in emergency officials, and that could be exacerbated by using social media without their involvement and consultation in its approach and implementation. This is because there’s a lot of misinformation on these platforms.

There’s also the question of who is using these platforms and who has access, which leads to literacy and access challenges which could also lead to an underrepresentation of vulnerable communities in emergency communications.

Through a study, researchers looked at who actually tweets in disasters scenarios, and it showed that it works for the people that are physically vulnerable (people in the physical path of the disaster), but not necessarily good for the socially vulnerable. By using these different platforms and methods of using data in your response, it could create a widening gap in care.

A few takeaways

If you’re going to use social media, understand that Twitter and any social media isn’t a neutral platform, and it doesn’t represent the whole population. Public education needs to be used before a disaster on a sunny day to teach people how you want them to interact with the platform or different tools that you’re trying to use.

Address the issue of “does the Disaster Management Team have the capacity and staff capable to handle the information coming in?”.

If you’re getting this max influx of messaging that you can’t handle, you will then violate that trust of your vulnerable communities. This is really delicate in this field of work.

Lastly, you can’t just rely on any of any of the things that I’ve explained. You can’t rely on it because if there’s an electricity grid outage, all of them are dependent on that. If the electricity grid is taken down and you were only relying on these tools, then you would be creating a larger vulnerability for yourself.

Violence Knows No Borders — Global Issues

The detained soldiers were at the Merowe air base in northern Khartoum, run by Sudanese armed forces – officially for no more than an agreed training mission. However, the RSF and international observers assessed their presence as long-term support for the Sudanese army led by General Burhan.

For many Egyptians, these humiliating images produced a rally ‘round the flag effect. In a tragic way, they pointed to not only the geographical proximity but also the bond between the two neighbours.

In addition to the over 1,200-kilometre-long border in southern Egypt, the two countries have a multi-layered relationship visible not only in a common colonial past but also in the political developments of the past decade.

The outbreak of conflict between the RSF militias and the Sudanese army put an abrupt end to political developments led by the Sudanese military – a scenario with which the Egyptian leadership was comfortable.

In any case, Egypt has no interest in a destabilisation of Sudan and is calling on all sides for a permanent ceasefire. Political uncertainties on the country’s southern border and Sudan sinking into civil war are a worst-case scenario for Egypt. Sudan, which borders the Nile, is not only an important ally for Egypt on the issue of the Grand Ethiopian Renaissance Dam (GERD) project.

The Egyptian leadership is betting on the Sudanese military, seen by them as the best guarantor of their own interests.

The geopolitical jigsaw puzzle of different supporters for the two sides in the Sudanese conflict has placed Cairo in a diplomatic quandary. The United Arab Emirates (UAE), an important Egyptian ally, also supports the RSF, as does Libyan General Haftar, a close ally of Cairo in the conflict in Libya.

Egypt, which is right now in a severe economic and financial crisis, is dependent on investment from the Gulf region and in particular from the UAE. The conflict in Sudan has therefore a domestic political dimension as well. All this makes the conflict difficult for Egypt’s diplomacy to navigate.

Egypt can play an important and constructive role in the short term, in particular in persuading General Haftar not to deliver weapons from Libya to the crisis region. In the long term, it will be crucial for the region’s stability for civil actors to be involved in resolving the conflict in Sudan.

This should also be in Egypt’s long-term interest as it could avoid the scenario of an ongoing civil war. The situation on the Sudanese-Egyptian border is very tense at the moment. Thousands of refugees are making the long and costly journey from Khartoum to Cairo.

The few border crossings between Egypt and Sudan are the bottlenecks through which thousands of Sudanese refugees squeeze their way. The unresolved border conflict between the two countries in the Halaib Triangle further reduces the number of possible crossing points.

The United Nations High Commissioner for Refugees (UNHCR) estimated some 50,000 people to have crossed the border into Egypt in recent weeks. Even before the conflict broke out, around five million Sudanese were living in Egypt, most of them already for tens of years. Of these, just some 60,000 are registered with the UNHCR as refugees.

The Sudanese community – especially in Cairo – is therefore an important contact point and is playing a central role in taking in the refugees. In the long term, however, this will not be feasible. The longer the conflict lasts, the more important it will be to stand by Egypt and the UN organisations working in Egypt to support the refugees.

With all attention focused on the successful evacuation of international personnel from Sudan, the protection and support of the Sudanese refugees in Egypt and the other neighbouring countries must not be forgotten.

South Sudan

More than 30,000 people have fled Khartoum and the western Darfur region across the South Sudanese border since fighting broke out. The vast majority of them are South Sudanese, who, in the past, sought refuge in the north from violence and conflict south of the border.

The small border town of Renk, where most of those fleeing Khartoum arrive, is overwhelmed by the sheer amount of people. Humanitarian organisations are trying to get those arriving at the small airport in Paloch to continue their journey to Juba as quickly as possible.

But there are not enough flights, so, to prevent a humanitarian crisis in the border area, the government is now considering using barges on the Nile to take the refugees to other parts of South Sudan.

The arrival of tens of thousands of refugees is the most visible impact of the war between the two Sudanese generals on the country’s southern neighbour. The two Sudans remain deeply interlinked even after South Sudan gained independence in 2011.

But what worries the government in Juba most is the risk that Sudan’s oil export infrastructure will be deliberately destroyed in the course of the fighting. More than 90 per cent of South Sudan’s government revenue is generated from oil production. The ‘black gold’ reaches the international markets via Sudan. There is no alternative pipeline.

Currently, the Sudanese National Army is reportedly controlling the revenues from the pipeline user fees, no doubt to the chagrin of the RSF militia. An attack on the pipeline would have devastating economic consequences for Juba.

The responsible ministry is already complaining that the market price for South Sudanese oil has fallen owing to the situation in Sudan, with buyers taking advantage of the instability to drive down prices.

The joint interest of the South Sudanese government and the Sudanese army under General Burhan in maintaining oil exports is in turn likely to impact South Sudanese President Salva Kiir’s peace efforts. He has invited both parties to the conflict to mediation talks in Juba, so far without success.

South Sudanese activist Edmund Yakani warns that the crisis in Sudan is also endangering the peace process in South Sudan. Sudan is one of the two regional guarantors of the peace agreement that formally ended the civil war in 2018 and the actor with the most powerful leverage over the peace parties in Juba.

There is now a danger that the parties will continue to delay the peace process – also with the attention of the international community and other regional players now focused on Sudan. In addition, the instability in Sudan threatens to flush even more small arms across the border, while the brisk trade in everyday goods between North and South is suffering, further worsening the precarious supply situation.

All parties to the South Sudanese peace treaty, as well as armed groups fighting the transitional government, have links with various civilian and armed players in Sudan. How the conflict ends and who ultimately gains the upper hand will directly impact power dynamics in the political mêlée in Juba and the conflict landscape in the wider country.

Should the conflict continue and widen, both sides could also recruit keen-to-fight forces from South Sudan’s fragmented conflict landscape, who could later return with more weapons, combat experience and new external political backing.

Finding Ways to Feed South Africas Vast Hungry Population — Global Issues

It’s in villages like this one that the stark statistics of one in five South Africans being so food insecure they beg to feed themselves and their families could be a reality.

The village instead supports its fragile community through an agroecological project, Abanebhongo People with Disability (APD), co-founded in 2020 by Nosintu Mcimeli as an example of food sovereignty in action.

Food security in South Africa, the second wealthiest country by GDP, is low. According to 2019 data, Statistics SA says at least 10 million people didn’t have enough food or money to buy food.

Impacts on Physical Development, Mental Health

The impacts of this are devastating; hunger not only impacts physical development but also people’s mental health. Siphiwe Dlamini, writing in The Conversation, recently reported on a study that found that those who could not afford proper nutrition resorted to eating less, borrowing, using credit, and begging for food on the streets, which was the most harmful coping strategy for mental health.

“We found that over 20% (1 in 5) of the South African households were food insecure. But the prevalence varied widely across the provinces. The Eastern Cape province was the most affected (32% of households there were food insecure). We also confirmed that food access in South Africa largely depends on socioeconomic status. People who are uneducated, the unemployed, and those receiving a low monthly income are the most severely affected by inadequate food access,” wrote Dlamini, a lecturer School of Physiology, University of the Witwatersrand.

The situation in the region is also dire, with a UN World Food Programme (WFP) report in 2020 revealing that 45 million people were severely food insecure in the Southern African Development Community (SADC).

South Africa has long been afflicted with widespread hunger, but the onset of Covid, an ailing economy, climate change, fuel and food price increases, interest hikes, and the impact of the Russia-Ukraine war has deepened the food crisis.

However, Vishwas Satgar of the SA Food Sovereignty Campaign (SAFSC) says even before Covid, the number of hungry people was close to 14 million – and “women shoulder the burden of the high food prices, sharing limited food, skipping meals, and holding families together.”

The irony, Satgar says, is that the country can feed all its people.

“We produce enough food, but it’s essentially for export. The stark paradox in the commercial food system is that it is just another commodity; most people can’t feed themselves. The poor eat unhealthy (cheaper) food, and we have an obesity problem.”

Satgar says a change of strategies is needed to feed the poor.

“Despite overwhelming research proving that small-scale farmers feed the world, many people have the perception that large-scale industrial farms are the ultimate source of food. South Africa, with an expanded unemployment rate of 46.46 percent (start of 2022), cannot afford to lose more farm workers. Agroecological farming can transform the rural and urban economy with localised farming practices that absorb many unskilled and semi-skilled people,” he says.

The SAFSC, the Climate Justice Charter Movement, and the Cooperative and Policy Alternative Centre (COPAC) are building a new food system to avert a catastrophe.

Food Sovereignty 

“We call this the food sovereignty system, which is democratically organised and controlled by small-scale farmers, gardeners, informal traders, small-scale fishers, communities, and consumers.

That’s where Mcimeli comes in. She tells IPS her activism journey began after she left a company that worked with people with disabilities in Cape Town. She contracted polio as a baby because her domestic worker mother could not take her for immunisation. “I have a disability in my right thigh and leg.”

She was working as an informal trader when she was given the opportunity from SADC, “which was releasing millions of rand to train SA women for activism in any kind of project.”

Mcimeli was one of 80 women trained in 2012 and 2013.

“In 2014, I was transferred to Copac for activist schooling. That’s when I met Vish (Satgar). I then decided to come to the Eastern Cape to plough back my activism skills.”

It was here that she co-founded the APD, and it has become an example of food sovereignty in action in Jekezi in the Eastern Cape.

Mcimeli says the ADP started an agriculture project.

“Because in rural areas there is communal land, it’s free, so we formed groups to start communal gardens. Then I realised that there are people who are bedridden, so I started enviro gardens in nearby villages. At the moment, we have 24 of these, and they are working.”

She works with four young women but wants to include more young people in the projects.

Forever Water—Free and Healthy

During the hard lockdown, the ADP got a big water tank from the local municipality and started a soup kitchen.

“We got donations of masks and sanitisers and food from Shoprite. Then a colleague of mine organised radio interviews for me, and a company that provides boreholes heard me asking for more water tanks. They said they had a lifetime solution and sponsored a community borehole. It was installed free of charge in a local schoolyard. It’s forever water—free and healthy and available for everyone, not just our projects”.

One of ADP’s beneficiaries, Bonelwa Nogemane, says: “I have a family of seven including a disabled four-year-old; we are often hungry because the food is too expensive. I joined the ADP to help my family and community to grow our own food.”

While the ADP is making a small dent, the problem is much bigger, and activists warn that unless a solution is found to the hunger crisis, South Africa is in danger of producing a lost generation of intellectually and physically stunted future leaders.

A study published in BMC Public Health on the link between food insecurity and mental health in the US during Covid found that: “Food insecurity is associated with a 257% higher risk of anxiety and a 253% higher risk of depression. Losing a job during the pandemic is associated with a 32% increase in risk for anxiety and a 27% increase in risk for depression.”

Campaign to Save Children from ‘Slow Violence of Malnutrition’

Marcus Solomon of the Children’s Resource Centre, which has launched a campaign to save SA’s children from the “slow violence of malnutrition”, says: “The consequences of this are dire for the affected children, with an estimated four million children in SA having stunted growth because of malnutrition and another 10 million going hungry every day.”

Activist Shanaaz Viljoen from Cape Town says: “My personal experience on a grassroots level is rather heartbreaking. The children we work with are always hungry due to the situation in their homes.”

In addition to an alternate food system, Trade Union Federation Cosatu, the SASFC, Copac, and others believe introducing a Basic Income Grant will go a long way towards addressing the hunger crisis in the country.

Parliamentarians Ask G7 Hiroshima Summit to Support Human Security and Vulnerable Communities — Global Issues

The wide-ranging declaration also called on governments to support active political and economic participation for women and girls, enhancing and implementing legislation that addresses gender-based violence (GBV) and eradicating harmful practices like child, early, and forced marriages. During discussions and in the declaration, a clear message emerged that budgetary requirements for Universal Health Care (UHC) should be prioritized and the exceptional work done by health workers during the pandemic be recognized.

In his keynote address, Japan’s Prime Minister Kishida Fumio reminded delegates that Covid-19 had exposed the “fragility of the global health architecture and underscored the need for UHC.”

Kishida said that the central vision of the G7 Hiroshima Summit was to emphasize the importance of addressing human security – through building global health architecture, including the “governance for prevention, preparedness, and response to public health crises, including finance. We believe it is important for the G7 to actively and constructively contribute to efforts to improve international governance, secure sustainable financing and strengthen international norms.”

Apart from contributing to resilient, equitable, and sustainable UHC, health innovation was needed to promote a “more effective global ecosystem to enable rapid research and development and equitable access to infectious disease crisis medicines … and to support aging society,” Kishida said.

Former Prime Minister of Japan Fukuda Yasuo, Chair of APDA, and Honorary Chair of JPFP said this conference and its declaration would follow in a tradition of delivering strong messages to the G7 that improving reproductive health was crucial to the development and the future of a planet which now had 8 million people living on it.

“International Community is becoming increasingly confrontational and divided, and there is the emergence of a national leader who is threatening the use of nuclear weapons. No nuclear weapons have been used in the nearly 80 years since Hiroshima and Nagasaki. We must work together to prevent the use of nuclear weapons, which can take many precious lives and people’s daily lives. In this instance, I would like you to search for the path toward appeasement and not division. We must keep all channels of dialogue open so as to ease tension,” Fukuda asked of the conference.

While calling on parliamentarians to work together to address challenges, Fukuda also expressed concern about the widening inequities caused by Covid-19 and climate change and noted: “This network of parliamentarians on population and development has been a vital resource for parliamentarians who share the same concern for not only their own countries but for the entire planet and future generations.”

Kamikawa Yoko, MP Japan, Chair of JPFP, said that with a world population of 8 billion, it was essential to “realize a society where no one is left behind … and Japan would share its experiences of being on the frontlines of an aging society with declining birth rates. “We are living in an aging society … and given these challenges in Japan, we will try to share with you our experience and lessons through our diplomacy while trying to deepen our discussions and exchanges to seek solutions.”

Japan’s Foreign Affairs Minister Hayashi Yoshimasa said it was essential for all to cooperate during the “Anthropocene era, when human activities have promised to have a major impact on the global environment, global issues that transcend national borders, such as climate change, and the spread of infectious diseases, including Covid-19 are becoming more and more prevalent.”

He reminded the delegates that at the center of Japan’s economic growth post World War II was mainly through health promotion and employment policies.

Director of the Division for Communications and Strategic Partnerships of UNFPA, Ian McFarlane, said it was not about the “numbers of people but the rights of the people that matter. It’s not about whether we are too many or too few, but whether women and girls can decide if, when, and how many children to have.”

A recent UNFPA report indicated that nearly half of the women across the globe could not exercise their rights and choices, their bodily autonomy, and expressed hope that policies in the future continue to focus on humanity and universal human rights.

Despite being close to the 30th anniversary of the International Conference on Population and Development (ICPD), the conference heard that much still needed to be done regarding women’s rights.

New Zealand MP and co-chair of AFPPD Standing Committee on Gender Equality and Women Empowerment, Angela Warren-Clark, reminded the audience that women still only held 26 percent of parliamentarian seats globally. While women make up 70 percent of the workforce in the health sector, only 25 percent have senior leadership positions.

“It is women in this pandemic who bore the increased burden of unpaid work at home as schools were closed, and it is girls and the poorest families who were taken out of school and forced into early marriages … We believe that if women had an equal say in decision-making during the pandemic, some of these mistakes would have been avoided.”

Baroness Elizabeth Barker, MP from the United Kingdom, told parliamentarians their role was to ensure that “no person on earth, from the head of G7 country to a poor person in a village, can say that they do not know what gender equality is. And they do not know what gender violence is.”

Barker suggested they use international standards, like the Istanbul Convention on Violence Against Women, to compare countries. “And you know that if your country doesn’t come out very well, they really don’t like it.”

She pointed to two successes in the UK, including stopping virginity testing and tackling the practice of forced marriages. She also warned the delegates that there was a right-wing campaign aimed at destroying human rights gained, and they chose different battlegrounds. The overturning of abortion rights in the United States in the Roe vs. Wade case was an example, as was the anti-LGBTQ legislation in Uganda.

Hassan Omar, MP from Djibouti, gave a host of achievements in his country, including ensuring that women occupy 25 percent roles in politics and the state administration and the growing literacy of women numbers in his country.

Risa Hontiveros, MP Philippines, painted a bleak picture of the impact of Covid in her country.

Hontiveros said GBV increased during Covid and extended to the digital space.

“The Internet has become a breeding ground for predators and cyber criminals to prey on children, especially young women, and girls. The online sexual abuse and exploitation of children … has become so prevalent in the Philippines that we have been tagged as the global hotspot.”

In a desperate attempt to provide for their families, even parents produced “exploitative material of their own children and sold them online to pedophiles abroad.”

To address these, she filed a gender-responsive and inclusive Emergency Management Act bill, which seeks to address the gender-differentiated needs of women and girls, because they were “disproportionately affected in times of emergencies.”

Former MP from Afghanistan Khadija Elham’s testimony united many in the conference and even resulted in proposals from the floor to include a condemnation of the Taliban’s women’s policies.

Elham said GBV had increased since the Taliban took over – women were forced to wear a burqa in public, they were not allowed to work, and those who wish to “learn science or (get an) education are forced to continue their studies and hidden places like basements.”

If their secret schools are exposed, they face torture and imprisonment. During the last two months, 260 people, including 50 women, were publicly whipped – a clear violation of their human rights. Women’s representation in political life has been banned, and women are no longer allowed to work in NGOs – and it has been “550 days since women could attend high schools and universities.”

She called on the international community, the United Nations, to pressure the Taliban to restore women’s work and education rights.

Nakayama Maho, Director of the Peacebuilding Program at the Sasakawa Peace Foundation, announced new research on factors contributing to men’s propensity to GBV. The research found that the higher a man’s educational attainment, the lower the level of violence. There were also lower levels of violence with “positive” masculinity – such as a man being employed, married, and capable of protecting his family. Men who experienced violence during times of conflict tended to support violence to instill discipline, or protect women and communities.

Dr Roopa Dhatt, Executive Director of Women in Global Health, summed up this critical session by saying, “Equal leadership for women in all fields is a game changer, particularly in politics and health.”

Japan’s Health, Labour and Welfare Minister, Kato Katsunobu, noted during his closing address that the G7 countries “share the recognition that investment in people is not an expense, but an investment… and as you invest in people you can create a virtuous cycle between workers well-being and social and economic activities.”

He said Japan had a lot to offer concerning aging populations.

“Japan has been promoting the establishment of a comprehensive community-based care system so that people can continue to live in their own way in their own neighborhood until the end of their lives and is in the position to provide knowledge to the G7 countries and other countries who will be facing (an aging population) in the future.”

Dr Alvaro Bermejo, Director-General of IPPF, commended the conference and said he was “thankful” that the conference declaration would tell G7 governments to set an example. “Marginalized and excluded populations are at the heart of human security and can only be achieved in solidarity, and that message from this conference is clear.”

Professor Takemi Keizo, MP Japan, Chair of AFPPD, summed up the proceeding by saying that parliamentarians as representatives of the electorate were vital to creating a “positive momentum in this global community and overcoming so many difficult issues.”

Takemi elaborated on some issues facing the world now, including climate change and military conflicts, but as parliamentarians, there was the opportunity to “build up the new basis of the global governance, which can be very beneficial.”

New Mosquito Species Could Derail Fight Against Malaria — Global Issues

According to a report by experts from the Kenya Medical Research Institute (KEMRI), the species was first noted during routine mosquito surveillance in Saku and Laisamis villages in Marsabit County. The report states that, unlike the traditional mosquito vector, the Anopheles stephensi can adapt to man-made habitats that include plastic containers, discarded car tyres and open sewer lines—this makes urban centres a hot spot for their prevalence.

Anopheles stephensi is endemic to South Asia and Arabian Peninsula, where it is a known carrier for two malaria variants Plasmodium falciparum and P. vivax. It was first noted at the Horn of Africa ten years ago in Djibouti, after which it was later tracked down in Ethiopia, Somalia, and Sudan in 2019.

The species is also known to survive through different climatic conditions, which can enable it to cause problems all year round if left uncontrolled.

‘’This mosquito most likely spreads through ships coming in from Asia since genetic analysis of many of the samples collected in Africa shows they are closely related to those found in Asia. Once they got to Africa, it is highly likely they have been transported southwards on the road,’’ said Dr Eric Ochomo, the project’s lead researcher and an entomologist at the KEMRI, Kisumu.

‘’It breeds in a wide range of habitats, mostly water storage containers that are not covered, manholes, overhead tanks, poorly dumped plastic containers etc.’’

Malaria has been a perennial problem in Kenya and Africa, given the vast tropical conditions that favour mosquitos and unreliable health facilities that make its control and treatment an almost impossible hurdle.

While being a nuisance in Africa, most malaria cases and mortalities have been recorded in rural areas, characterized by a lack of adequate medical amenities, unreliable infrastructure, and a lack of knowledge among residents.

Urban areas have usually been spared the malaria burden due to access to proper medical facilities and a good understanding of the disease and how to control and prevent it.

This notion may, however, change for the worse as this new mosquito species threatens the demographics and steps made in the fight against malaria in Africa.

‘’This species is different from the traditional mosquito for two main reasons; A) its diversity of breeding habitats means it can breed in rural and urban settings alike, which means that it is not restricted to rural habitats like the Anopheles gambiae, Anopheles arabiensis and Anopheles funestus which are the most common vectors in Kenya at the moment. B) It can transmit both Plasmodium falciparum and Plasmodium vivax parasites. We currently have very low levels of P. vivax transmission in Kenya, and this could be increased by this vector,’’ Ochomo explained to IPS.

Despite the 2020 world malaria report showing a significant decrease in malaria deaths over the past two decades (from 84 percent in 2000 to 67 percent in 2019), it remains one of Africa’s leading causes of death, especially among pregnant women and children under the age of five.

The report stated that 51 percent of the global malaria deaths were in Africa, with Burkina Faso (4%), the Democratic Republic of the Congo (11 percent), Nigeria (23 percent), Mozambique (4 percent), Niger (4 percent) and Tanzania (5 percent).

In Kenya, most malaria cases are centred around the malaria endemic areas, including the coastal and lake regions, which form prime breeding spots for female anopheles mosquitos. For the cases reported in towns such as Nairobi, a follow-up on the patient’s movements often reveals that they recently visited or through one of these malaria-endemic places and got infected.

According to the Centers for Disease Control (CDC), about 3.5 million malaria cases are reported in Kenya annually, with about 10,700 mortalities. Out of this, western Kenya (lake region) usually records the highest number of cases at 45 percent.

The lake and coastal regions are categorized as malaria-endemic due to the favourable temperature and humid conditions they provide for mosquito breeding.

With most people in central Kenya and the highland areas having little exposure to malaria infections, this new vector could prove problematic given their immune system’s primitiveness to the disease.

The 2020 Kenya malaria indicator report says that low-risk malaria areas include Nairobi, Nyandarua, Nyeri, Kirinyaga, Murang’a, Kiambu, Machakos, Makueni, Laikipia, Nakuru, and Meru. Most of these areas are considered urbanized compared to most parts of Kenya.

Seasonally, areas that experience malaria outbreaks include Tana River, Marsabit, Isiolo, Meru, Garissa, Wajir, Mandera, Turkana, Samburu, Baringo, Elgeyo Marakwet, Kajiado. This is mainly due to the arid and semi-arid conditions experienced throughout the year that do not favour mosquito breeding.

‘’What this means is that we are going to have more incidences of malaria because this vector can thrive in both rural and urban settings and many other geographical regions,’’ says Dr Alex Owino, Medical Superintendent, Katulani Sub-County Hospital, Kitui.

‘’Kitui county falls under the low-risk malaria areas, with the few cases recorded being mainly from patients who had recently travelled outside the county,’’ he told IPS.

Owino explained that controlling malaria was easy when there were specific places where the host mosquito was known to favour. However, with this new vector being able to spread widely, it becomes a threat to the efforts made in the fight against malaria.

Being a developing country, parts of urban Kenya are characterized by poorly planned housing facilities, inadequate drainage systems and poor waste disposal management. Nairobi, for instance, is also known for hosting the largest slum in the country, Kibera, coupled with the Nairobi dam, which has, for years, made the headlines for having all manner of pollution destroying it.

All these conditions have been a recipe for various diseases, such as cholera and typhoid, causing health problems, especially in the slum areas. Now, malaria may have just added to the burden that these town dwellers have to deal with.

Ochomo said that, unlike the traditional malaria-causing mosquitoes, Anopheles gambiae and Anopheles funfests, the Anopheles stephensi is an invasive species that could bring malaria transmission to these areas where there is a large number of naive (have never had malaria) individuals.

‘’These individuals could get far more severe symptoms than people who have been exposed since birth,’’ he told IPS.

Wilson Opudo, a public health and infectious diseases specialist, also believes that the ongoing changes in climate conditions are likely to increase the malaria burden by creating mosquito breeding zones in areas where they were not a concern.

‘’Despite malaria being known to favour certain parts of Kenya, the recent changes in climate which have resulted in temperature increase and hydrological changes may help form new areas for the malaria vector breeding thus bringing malaria to places where it initially did not exist,’’ Opudo told IPS.

‘’This will put a lot of pressure on the malaria control commodities currently available for the endemic areas of Africa and could result in increased disease burden,’’ he added.

Ochomo concluded that its presence in urban settings means controlling this new vector will rely on properly managing waste disposal, covering water containers, and draining stagnant water.

‘’There is very little information available on the behaviour of the adult mosquitoes and an urgent need to invest in the research on this to inform what control methods would be applicable for the adult mosquitoes,’’ said Ochomo.

Can a Pledge to End TB Stick This Time Around? — Global Issues

    Meanwhile, the head of the World Health Organization (WHO), Tedros Adhanom Ghebreyesus, has unveiled plans to speed up the licensing and use of effective novel vaccines against tuberculosis (TB), the second leading infectious killer after COVID-19 and the 13th leading cause of death worldwide. TB is a disease that strikes hardest in impoverished places, and the last U.S. outbreak was no different. Disadvantaged urban communities hit hard by the HIV/AIDS pandemic bore the brunt of the outbreak.

Yet, at the outbreak’s peak in 1992, less than 27,000 people in the U.S. were infected with TB. Today, an estimated 304,000 people are infected with TB in South Africa every year, while just under 3,000,000 people are infected with TB in India. The scope of the disease today far exceeds what the U.S. saw three decades ago, much less what it sees now.

TB deaths have risen across the world for two consecutive years; at this point it kills more people than COVID-19. Globally, an estimated 10.6 million people were infected with TB, but only 6.4 million people were diagnosed. The other 4.2 million people infected with this potentially lethal and debilitating disease slipped through the cracks.

In 2018, the UN held the first high-level meeting on TB. More than half of the UN member states sent delegations and 15 heads of state spoke at the event.

In a resolution endorsed by the entire UN General Assembly, every nation pledged to invest, by 2022, a total of US$2 billion annually for TB research and US$13 billion annually for TB diagnostics, treatment and care. Other commitments—to treat more people and prevent more active disease—were also made.

The world got off to a slow start on meeting these pledges and then the pandemic hit. Many of the goals were not reached. We were delighted, for example, that the annual research budget for TB research finally reached US$1 billion in 2021, because it was a critically important achievement—but the actual goal was twice that amount.

And less than half of what was pledged annually on diagnostics, treatment and prevention—US$5.4 billion—was actually provided in 2021.

There are bright spots in the fight against TB, of course. Sub-Saharan Africa is the only region that did not suffer a significant slump in TB detection during the COVID-19 pandemic. But with just under 40% of active TB infections unidentified, it still lags behind much of the world in diagnosing active cases.

Another recent highlight is the approval of new drug-resistant TB treatments that can reduce treatment time from as much as a year and a half (or sometimes even longer) to six months—including one developed by my organization, TB Alliance. African nations like Nigeria and South Africa are taking steps to rolling out this new regimen so that the spread of drug resistant infections can be curbed.

The goal pledged at the 2018 UN meeting was that, between 2018 and 2022, 1.5 million people with drug-resistant TB (DR-TB) would be treated—this is a critical slice of the TB caseload. Yet only 649,000 DR-TB patients were treated between 2018 and 2021.

There is optimism that the number of DR-TB patients who can be treated will escalate, given the new treatments, but funding and resources must increase.

The bottom line is this: TB was the most lethal infectious disease before the COVID-19 pandemic, and as COVID recedes, it is once again the worst. As leaders prepare to meet in New York City and start drafting a new set of promises in the form of the next Political Declaration on TB, we need the world to commit to ending this disease, which has killed too many people for far too long.

But this time around, we also need the world to follow through on its commitments.
